Hot or cold showers for eczema? The advice everyone repeats has no study behind it

The part everyone with eczema knows
If you have eczema, you know the feeling. Hot water on itchy skin is one of the most satisfying sensations there is — for about thirty seconds, it's better than scratching. Everyone with eczema discovers this on their own, usually as a kid, and nobody has to teach it to us.
And you know the second part too: it doesn't last. For me it has never once lasted. The relief is momentary, and after the shower my skin is reliably worse — tighter, drier, itchier by the evening. I've been through that loop more times than I can count.
Here's a pattern from my own log that took me longer to notice: my skin is consistently better in summer. Part of that is obvious — sun, sea water (I've written about that one) — but part of it, I'm fairly sure, is simply that in summer I don't crank the shower anywhere near as hot. I can't cleanly separate those variables, and I won't pretend I can. A pattern in my log is a pattern, not proof. But it made me want to know what the research actually says about the thing I do every single day.
What I found genuinely surprised me: the advice about water temperature is the least-evidenced part of the entire eczema routine — and the part everyone repeats with the most confidence. Including me.
What temperature should the water be? Depends which country you ask
Every eczema resource on the internet says “lukewarm”. Almost none of them tell you what lukewarm means, and the official guidelines that do commit to a number can't agree:
- The European guideline (EuroGuiDerm 2022) and Korean guidelines say 27–30 °C (guideline PDF).
- The Japanese guidelines say 36–40 °C — reasoning that itch fires at around 42 °C and just below that is optimal for the skin barrier (ADGL 2021).
- The American Academy of Dermatology refuses to give a number at all. Their guidelines say it openly, three editions in a row: no standard for bathing frequency, duration, or water temperature “can be recommended on the basis of the limited available evidence” (AAD guidelines).
- The UK's NICE guideline doesn't mention temperature at all.
That's a ten-degree gap — the difference between a chilly shower and a warm bath — and both ends of it are published as official recommendations. The reason they can't agree is simple: no controlled trial has ever tested water temperature in eczema. Nobody has ever taken people with eczema, assigned them different shower temperatures, and measured what happened. Every number above is committee consensus or extrapolation from physiology. The AAD is just the only one saying it out loud.
What was actually measured: the villain is the soap, not the tap
The closest thing we have to a real experiment on this is from 1996, and its result is not what you'd expect.
Researchers took 20 volunteers and treated their forearms — randomised — with 20 °C or 40 °C water, ten minutes twice a day (Ohlenschlaeger 1996). The twist: they ran it both with plain water and with a detergent (SLS, the classic foaming agent). The result:
- With detergent: 40 °C water caused significantly more water loss through the skin than 20 °C.
- With plain water: no significant damage — at either temperature.
Read that again, because it quietly rewrites the standard advice: in the strongest experiment available, hot water on its own didn't do measurable harm — it did harm as an amplifier of the detergent. The action item hiding in “take colder showers” might actually be “look at what's in your shower gel”.
That maps onto my own experience more than any temperature rule does. What I wash with makes a bigger, more repeatable difference to my skin than a few degrees on the tap. I use Dexeryl's shower cream for atopic skin and it works for me; what I can say with more confidence is what doesn't work for me — perfumed gels and the cheap foaming stuff, which reliably leave my skin angry. Your mileage will vary, and that's the honest version: the evidence points at the detergent, not at one specific product.
One caveat to keep this honest in the other direction: a 2022 study had 50 healthy volunteers immerse their hands for ten minutes and found hot water (41 °C) increased water loss by 127% — but cold water (11 °C) also increased it, by 36% (Herrero-Fernández 2022). There is no magically “neutral” temperature. Water itself is a mild insult to a compromised skin barrier; heat amplifies it, and detergent amplifies it a lot.
Why a hot shower relieves itching — and why you regret it an hour later
The relief is real. It's not in your head, and the neuroscience behind it is some of the most solid science in this whole article. The heat receptor involved in itch (TRPV1) is causally wired into eczema itch — a drug blocking it went through a proper phase-3 trial with significant results (phase-3 RCT). Brief, intense heat genuinely does interrupt itch signalling: a 2024 study found that about five seconds of localised ~49 °C heat suppresses itch, repeatably.
But here's the mechanism of the trap, and it comes in two parts.
First: warming the skin makes itch worse. Raising skin temperature by a few degrees significantly increases histamine itch — researchers call it warmth hyperknesis. A five-second blast of heat interrupts itch; a fifteen-minute hot shower heats your whole skin up, which feeds it.
Second, and worse for us specifically: the heat-relief trick works differently on eczema skin. In a small but pointed study, repeated painful heat reduced histamine itch in healthy skin — but increased it in the lesional skin of people with atopic dermatitis (Ishiuji 2008). The very trick your nervous system learned actually inverts on the skin that itches most.
So the loop I've lived my whole life has a plausible explanation: the first seconds of hot water genuinely do kill the itch, which is why we all keep doing it — and the minutes that follow set up the evening rebound. We're confusing a five-second stimulus with a fifteen-minute exposure. (The popular explanations you'll see online — “the heat exhausts your histamine supply”, “the nerves get tired” — have no evidence behind them, by the way.)
How often should you shower with eczema? The war ended this year — in a draw
Here's where I have to correct something I've been telling people for years.
If you'd asked me last year, I'd have said with total confidence: one shower a day, maximum. More showers strip the skin barrier. It felt obviously true — it's mechanistically plausible, and every eczema forum repeats it.
Then the trial came out. A pragmatic randomised trial published in the British Journal of Dermatology in 2026 took 438 people with eczema and assigned them to daily bathing (six or more times a week) versus weekly (once or twice), for four weeks (Bradshaw 2026). The difference in eczema severity: −0.4 points on the POEM scale — statistically nothing (p = 0.30, on a scale where ~3 points is the minimum that means anything). The authors' conclusion: people with eczema should feel free to choose whatever frequency suits them.
So the “shower discipline” I was so sure about doesn't survive contact with the data. That stung a little, and it's also exactly the point of this article: on this topic, confidence and evidence are barely on speaking terms.
While we're breaking rules: the famous “three-minute rule” — moisturise within three minutes of stepping out or the shower was wasted — has no study behind the number. The tiny study (n=10) that gets cited actually found that moisturising immediately and moisturising with a delay were statistically indistinguishable — and that skipping the bath entirely and just moisturising beat both (Chiang & Eichenfield 2009). Moisturising after a shower is sensible; the stopwatch is folklore. So is “pat dry, don't rub” — repeated everywhere, tested nowhere.
Do evening showers ruin your sleep — or help it?
This one's personal. My working theory for years: a hot shower before bed heats me up, my body then spends the night cooling down instead of resting, I sleep badly — and bad sleep is terrible for my skin. That last link, at least, is real and well documented.
Then I checked the sleep literature, and it pushed back. A meta-analysis of 13 studies found that a warm shower or bath (40–42.5 °C) taken one to two hours before bed — even just ten minutes — shortens the time it takes to fall asleep by around 36% (Haghayegh 2019). The mechanism is the opposite of my theory: warm water pulls blood to your skin, which helps your core dump heat faster, and a falling core temperature is precisely the signal your body uses to initiate sleep.
But notice what those studies didn't measure: itch. They were done on people with normal skin. For us there's the warmth-hyperknesis problem from two sections ago — heating eczema skin right before bed is plausibly feeding the exact nocturnal itch that wrecks our sleep in the first place. A warm shower 90 minutes before bed and a hot shower right at bedtime might be completely different interventions for someone with eczema. Has anyone tested this in eczema? No. Add it to the pile.

Are cold showers good for eczema?
Given the cold-plunge boom, you'd think someone would have tested cold showers on eczema by now. Nobody has. There is not a single study. Everything you've heard is extrapolated from other contexts — and the extrapolation mostly runs backwards:
- A 2025 meta-analysis of cold-water immersion (11 studies, 3,177 people) found that plunging into ≤15 °C water acutely raises inflammatory markers, not lowers them (PLOS One 2025). “Cold water fights inflammation” is the opposite of what the pooled data show, at least in the hours after immersion.
- The studies cited to support cold exposure for eczema are actually whole-body cryotherapy pilots — −110 °C air, three minutes, two small uncontrolled studies, 32 people total. Different intervention, no control groups.
- One genuine caution: cold urticaria — an allergy to cold, affecting roughly 1 in 2,000 people. Most who have it don't know until their first bad reaction, and full-body cold-water immersion is the most common trigger of the severe systemic form (clinical review). Rare, but worth knowing it exists before you take up ice baths.
- Cooling itchy skin briefly is a different story — the cooling receptor (TRPM8) genuinely suppresses itch signalling, which is why a cold compress feels so good. Proven mechanism; the actual home remedy, untested in eczema. Story of this entire article.
My own experience with cold water: I've used the cold plunge after saunas and I like what it does for me generally — but I've never systematically tested it on my skin. That changes soon: cold showers are on my list to run as a proper self-experiment this year, logged daily. I'll report back with data instead of vibes.
Hard water: the one part of your shower with real evidence
Ironically, the strongest evidence about “what comes out of your shower head” has nothing to do with temperature. A meta-analysis covering 385,901 people found children in hard-water areas have about 28% higher odds of eczema (systematic review). In a study of 1,303 infants, hard water was linked to substantially higher odds of visible eczema at three months old — independent of chlorine levels (Perkin 2016). And there's a mechanism: calcium binds detergent residues to your skin, raising water loss — worst in people with the barrier-gene (filaggrin) mutation. Note the pattern: even hard water does its damage through the detergent. It's the soap again.
Before you buy a water softener, though: a randomised trial of 336 children tested exactly that, and softeners did not improve existing eczema (p = 0.53) (SWET trial). Hard water may play a role in eczema developing; softening it does not treat the eczema you already have. Whether softeners can prevent eczema in high-risk babies is a genuinely open question researchers are still working on.
What they're selling you
The gap between “sounds scientific” and “was ever tested” is where shower products for eczema live:
- Emollient bath additives — tested properly: 482 children, twelve months, no meaningful benefit (BATHE trial). The UK's NICE guideline now says flatly: do not offer them (NICE CG57). Sold for decades first.
- Shower filters / “dechlorinating” heads — the number of clinical studies in eczema is zero. The marketing leans on chlorine physiology; the largest infant dataset found chlorine wasn't independently linked to eczema at all.
- Vitamin C shower heads — the chemistry is real (ascorbic acid does neutralise chlorine); the skin benefit has never been tested on anyone.
- Cold plunges for “inflammation” — see above; the meta-analysis measured the opposite.
- And one for the road: the hygiene argument for hot showers doesn't exist either. A crossover study found water temperature made no difference to bacterial removal when washing (Jensen 2017). You don't need hot water to get clean.
How to find out what your skin actually thinks of your shower
Here's what I take from all of this. The research can't tell you your ideal shower — it can barely agree on lukewarm. But your shower is also one of the few eczema variables that is perfectly testable at home: you control the temperature, the length, the gel, and the time of day, every single day, and you can change any of them tomorrow at zero cost. Almost nobody actually tests them — we just inherit rules and repeat them confidently. I did, and one of mine didn't survive this article.
The method is the same one I've used on drinking water and everything else: change one thing — cooler showers, a different gel, moving your shower to the morning — and log your skin daily (intensity and itch, 1–10) for two weeks. Then look at whether they line up. Your memory will edit the story afterwards; a daily log won't.
A notes app or spreadsheet works fine. I built Exemio because I got tired of doing this by hand — it logs a day in a few taps and surfaces the correlations for me — but the method matters more than the tool.
